NEW GDMT OF HEART FAILURE WITH REDUCED EJECTION FRACTION (HFrEF): THE ROLE OF DAPAGLIFLOZIN

چکیده

Gagal jantung masih menjadi salah satu penyakit dengan angka mortalitas yang tinggi di seluruh dunia. Keterlibatan komorbiditas dan variasi etiologi dari ini merupakan tantangan bagi klinisi untuk melakukan pendekatan diagnosis serta penatalaksanaan pada gagal jantung. Klasifikasi itu sendiri terbagi tiga yaitu, Heart Failure with Preserved Ejection Fraction (HFpEF), Mildly Reduced (HFmrEF) (HFrEF) berdasarkan fungsi ejeksi fraksi. Penatalaksanaan selama mengarah kepada penghambatan jalur sistem renin angiotensin-aldosteron, neprilysin simpatis melalui agen angiotensin-converting enzyme (ACE) inhibitors, angiotensin II receptor blockers (ARB), receptor-neprilysin inhibitor (ARNI), beta blocker (BB) mineralcorticoid antagonist (MRA). Ketiga terapi kasus pilar dalam tatalaksana dengan/atau tanpa komorbid lainnya. Akan tetapi, penderita tetap meningkat secara global, sehingga lain dikembangkan beberapa studi. Studi terbaru menunjukkan pemberian diabetes mellitus yaitu Sodium-dependent Glucose Transporter-2 Inhibitor (SGLT2 Inhibitor) satunya dapagliflozin memiliki efek positif terhadap perbaikan klinis pencegahan komplikasi mellitus. Pada tinjauan pustaka akan disajikan pengunaan Dapagliflozin sebagai rekomendasi khususnya HFrEF.
